Provider-reported token accounting, in the fields every provider shares.

promptTokenCount is the standing prompt size — what to watch for context growth. totalTokenCount adds response tokens and does not describe it. Providers send more; the object passes through whole, so cast to the provider's own type (e.g. Gemini's LiveUsageMetadata) to read the rest. The raw counterpart of the normalized RealtimeLLMUsageEvent: a transport reporting both fires both from the same server message.
