Thrown by an OpenAI Realtime transport configured with cacheConfig.enforcePrefixStability: true when a connected, non-transfer updateSession() or transferSession() call would mutate the cache prefix (instructions or tools). Pre-connect mutations are always allowed; same- canonical-prefix calls do not throw.
